COVID-19 Safety Questionnaire *Required

If you are ill with COVID-19, or have been experiencing any flu-like symptoms, please notify us immediately and excuse yourself from set today.

If you have had contact with anyone with COVID-19 in the past 14 days, we ask that you let us know and excuse yourself from set today.

Should the previous not apply to you, please complete the below questions. If you answer “yes” to any of the below, or if you fail to answer any, we will need to excuse you from set today. We apologize for the inconvenience this may cause.

Have you tested positive for COVID-19 in the past 5 days?
Have you been in contact with a confirmed COVID-19 paitent within the past 5 days?
Have you experienced any of the following COVID-19 symptoms in the past 14 days? Cough, Shortness of Breath OR two of any of the following symptoms: Fever, Chills, Repeated shaking with chills, Muscle pain, headache, Sore throat, New loss of taste or smell?
Have you been in contact with a person who is currently being screened or under COVID-19 quarantine by a medical or governmental official?
Have you travelled to any place, internationally or domestically, that is considered high risk for COVID-19?

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Heath Organization. COVID-19 is extremely contagious and is believed to spread from person-to-person contact. As a result, federal, state, and local government and federal and state healthy agencies recommend social distancing.


We have put in place preventative measures to reduce the spread of COVID-19 as much as possible; however, we cannot guarantee that you will not become infected with COVID-19.

By signing below, you acknowledge that you have answered all the above questions truthfully and that you understand that coming to set is at your own risk.

 

Furthermore, if you become positive with COVID-19 following the shoot, you will notify
us immediately.
